PCHC, LLC is a home care provider who provides seniors, elderly, aged, sickly, disabled, and individuals with ailments, medical conditions, diseases, in our communities with home care assistance from our dedicated, competent, skilled, and professional home caregivers with the necessary provisions of Activities of Daily Living (ADL's).
PCHC is a non-medical home care service provider who assist older people, sickly, aged, disabled, and individuals with ailments, various medical conditions and chronic diseases to live independently in their own home by helping them with the tasks of everyday living. These services are non-medical in nature and are delivered by qualified personnel and are sensitive to the unique needs of the client and community. These services do not require the supervision of a registered nurse and do not require a physician's order. The tasks to be performed by our home care workers are non-medical in nature and do not require clinical judgment and may be performed by unlicensed individuals. We specialize in personal care and 24-hour live-in care.

We will take the time to get to know you and develop an individualized care plan that fits your specific needs. We understand that not one care plan fits all. Nursing assessments are conducted to identify the actual or potential health needs of individuals/families, which include the physical, medical, social, and emotional aspects of the assessed person's condition (s), the level of care indicated, referrals required and, if needed, coordination of care with other health professionals. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We offer all the necessary requirements for your love one to have a happy, healthy, productive, safe, prosperous day.
