Personalized in-home private duty home healthcare service provider who are available to assist seniors, elderly, aged, disabled, sickly, individuals in our communities that have ailments, medical conditions, and diseases with their home healthcare needs.
Medication Management
Our Skilled, trained medical personnel are medication reminders of your loved ones. "Did I Take My Pills?" Our certified HHA/CNA's are responsible for assisting your loved ones with self-administered medications. Because we are a non-medical organization, Medications are to be pre-poured by family into a pill box or hired nurse and our home caregivers are to remind and dispense, but not allowed to administer any medications to patients.
1-Hour Wellness Checks
PCHC home caregivers are allowed to check client blood pressure (BP),take the temperature of the client (TPR's; vital signs, if needed as well as turn and position bedbound, disabled client/ patient/ in bed that are immobile, also the maintenance of Catheter Care.
Wound Care
PCHC caregivers are not licensed to dress or care for patients wounds. This medical procedure is to be done by a skilled nurse professional or medical physician, as PCHC are a non-medical healthcare provider. We will assist with outside medical support in this needed area of care.
Fall Risk and Balance Assessment
The policy and procedure of PCHC fall risk management is to (1) call 911, (2) report any falls of a client immediately to the nursing office supervisor, (3) caregiver is not to pick the client up for risk of further injury, but wait for the paramedics to assess the client's condition due to the fall, (4) stay with the patient until paramedics arrive and keep the patient as comfortable and safe as possible, (5) notify the family.
Beware of the Five I's of the Geriatic client (elderly people)

(3) Incontinence
(4) Intellectual impairment
(5) Iatrogenic Complications
Geriatric Care Management: Provision of Information
Gerentology is the study of geritarics. Geriatircs relates to old people, especially with regard to healthcare. Geriatric care management is the process of planning and coordinating care of the elderly and others with physical and or/mental impairments to meet their long term care needs, improve their independence for as long as possible.
PCHC are equipped in managing the geriatric patient in a respectful, mannerable manner: PCHC care staff:
-Use proper form of address
-Establish respect right away by using formal language
-Take a few moments to establish rapport
-Try not to rush
-Avoid interrupting them
-Use active listening skills
-Demonstrate empathy

PCHC Information Provision:
PCHC ensures that current and potential clients/clients' representatives have access to comprehensive information, which will enable them to make informed decisions on whether or not the Agency can meet their specific requests and needs.
PCHC is a non-medical home care service provider who assist older people to live independently in their own home by helping them with the tasks of everyday living. These services are non-medical in nature and are delivered by qualified personnel and are sensitive to the unique needs of the client and community. These services do not require the supervision of a registered nurse and do not require a physician's order. The tasks to be performed by our home care workers are non-medical in nature and do not require clinical judgment and may be performed by unlicensed individuals.
HOME CARE VISITS AND FRIENDLY REASSURANCE
PCHC health care team will provide regular, friendly, Reassurance Services. Friendly Reassurance Services refer to making regular contact, through either telephone or in-home visits, with homebound individuals to assure their well-being and safety. Friendly Reassurance Services include contacting clients (via) or through telephone or by making in-home visits. Supervisor shall monitor Friendly Reassurance services by conducting evaluation follow-ups at regular intervals to assess the success of service delivery.
