General Care and Supervision
Services we offer is 24-Hour Live-in Care, Personal Care, Homemaker, Companion/Sitter, Respite Care, Intermittent Care, Friendly Reassurance, Chores, and assisting clients with moving from home to hospital or other healthcare facility and from hospital or health care facility to home. Transporting patients to doctors visits, errands, and outings.

Companion/ Sitter Services
PCHC Companion/ Sitter services shall be delivered in a caring and respectful manner, in accordance with relevant PCHC organizational policies and procedual standards.
Companion/ Sitter services include non-medical care, socialization, and basic supervision to ensure a clients safety and well-being. Companions may provide light housekeeping tasks, which are secondary to the care supervision of the client.
*CHORE SERVICES OFFERED:
Chore Services are short-term, non-continuous tasks necessary to mainatain a clean, safe, sanitary and inhabitable home environment to maintain clients' health and safety. Chore services include but not limited to cutting lawns, edging, weeding, pruning, clearing sidewalks of ice and snow, splitting and stacking firewood, raking leaves, planting gardens, mulching, fertilizing, trimming short trees and shrubs, replacing light bulb, fuses, electrical plugs, and frayed cords, and more.
*FRIENDLY REASSURNACE SERVICES:
Friendly reassurance services refer to making regular contact, through either telephone or in-home visits, with homebound individuals to assure their well-being and safety.
Personal Care and Hygiene
Personal Care Services Provided: They shall be delivered in accordance with State regulations.
Personal Care Services offer assistance to clients, who have experienced a loss in their ability to perform the activities of daily living, to remain in a safe, clean and healthy home environment.
Activities of Daily Living (ADL's) refers to activities, including bathing, dressing, transferring, using the toilet, eating, and walking, which reflects clients capacity for self care.

Light Housekeeping
HOMEMAKER SERVICES OFFER:
Homemaker services offer direct and practical assistance with household tasks and and related activities. Homemaker services assist the client who has experienced a loss in the ability to perform the instrumental activities in daily living to remain in a safe, clean, and healthy home environment. Instrumental Activities of Daily Living refers to certain daily tasks (light housework, shopping for groceries or clothes, using the telephone, and managing money), assist in reading mail, communication by writing or typing which enable clients to live independently in their community.

Transportation
PCHC offer assistance to your loved ones by public transportation accompanied with a qualified caregiver or clients may prefer to have caregiver drive their own (client's personal vehicle) in accordance with State guidelines and regulations (car insurance, handicap sticker and permission from a competent loved one). PCHC assist with making arrangements for your loved ones through public transportation if needed.
Our skilled transportation aides will assist with ambulation and transfers into appropriate transportation.
